血液浄化

敗血症・敗血症性ショック
  • 世界疾病負荷調査(GBD)のデータ解析では、2017年の敗血症・敗血症性ショックの年間症例は4,890万件、死亡は約1,100万件と推定され、全死亡の約20%を占めました。11
  • 特にIL-6などの血漿サイトカイン高値は、敗血症/敗血症性ショック患者のICU入室後の死亡リスク上昇と関連します。12-15
  • 敗血症/敗血症性ショック患者の高いエンドトキシン活性は、重症度16-18や臓器不全16の増加と関連します。
多臓器不全
  • 重症敗血症の重症患者の約75%で多臓器不全が報告されています。19

血液浄化療法の利点

血液浄化は、病因となり得るメディエーターや毒素を血液から除去するプロセスと定義されます1,31

血液浄化療法は、炎症性メディエーター、PAMPs(エンドトキシンなど)、DAMPs(HMGB1など)を除去し、不均衡な免疫反応の影響に対処することを目的としています1,33

敗血症/敗血症性ショックの患者では、血液浄化療法の使用によりノルエピネフリンの用量・投与速度の減少がみられるというエビデンスがあります34-38

敗血症性ショックとAKIの患者を対象にした無作為化クロスオーバー二重盲検試験では、炎症性メディエーターとエンドトキシン除去のための吸着フィルターセットを使用した持続的腎代替療法(CRRT)と血液浄化療法で24時間治療後、ノルエピネフリン投与速度はベースラインから87%低下しました(P= 0.02)38

血液浄化療法は、AKIの有無にかかわらず重症COVID-19患者で臓器機能(SOFAスコアで測定)の臨床的改善との関連が報告されていますことを示すエビデンスがあります39,40

前向き多施設共同観察研究では、炎症性メディエーターとエンドトキシン(PAMPs)を除去する吸着型フィルターセットを使用した血液浄化療法で治療した重症COVID-19患者において、SOFAスコアで測った臓器機能が改善(P=0.002)し、APACHE IVスコアによる予測死亡率と比べ観察死亡率が低下したことが報告されました39,40
