CRRTの多面的な利点を探る

CRRTの複数の利点には、血行動態および頭蓋内圧の安定維持、浸透圧シフトのリスク管理、体液貯留の効果的低減、そして多臓器不全への対応が含まれます。1,2 

エビデンスは、CRRTが間欠的療法と比較して短期・長期の透析依存リスクの低下と関連し、さらに長期コストの低減とも関連することを示唆しています。3-7

急性腎障害:患者ごとに異なる症状

急性腎障害(AKI)とは、7日以内に発生する腎機能の急激な低下として定義され、腎臓への直接的損傷と機能の急性障害の両方を包含します8

AKIは予後不良と関連する異質性の高い症候群であり8,9、各患者は固有のリスクプロファイルと疾患進行の軌跡を有します。10,11 


AKIは予後不良と関連するといわれています。

報告発生率の幅

ICU患者におけるAKIの報告発生率は0.5%から78.7%まで幅があります12-14。このうち最大で約25%の患者が腎代替療法(RRT)を必要とする可能性があります。15-18

死亡リスク

AKIは罹病率の上昇19-29および短期・長期の死亡率の上昇と関連します。30-35

CKDへの進行

AKIは、末期腎不全(ESRD)を含む慢性腎臓病(CKD)への進行リスクの上昇と関連します。12,33,36


重症AKI患者に対する基盤となる治療

腎代替療法(RRT)は、特に重症例においてAKI管理の主要な構成要素です。RRTには持続的(CRRT)と間欠的(IRRT)の2つのタイプがあります。持続的腎代替療法(CRRT)は、AKI管理における特定の臨床目標の達成を目的として、多くの臨床医に選択されています。37

血行動態

多くの臨床医は、血行動態が不安定なAKI患者に対してIRRTよりもCRRTを選択します。6,8

体液過剰の重症度

臨床ガイドラインは、体液過剰によって引き起こされる頭蓋内圧上昇または全脳性脳浮腫がある患者にCRRTを推奨しています。8

私たちは、血行動態が不安定な患者には、標準的な間欠的RRTではなくCRRTの使用を提案します。(グレード2B)8

体液管理のためのCRRT

CRRTは、体液貯留を効果的かつタイムリーに減少させることが示されており38,39、患者の臨床状態の変化に応じて、除水強度を調整できる柔軟性を提供します。8

限定的なエビデンスではありますが、CRRTは他のRRTモダリティより体液管理の制御に優れている可能性が示唆されています。8,38-40

血行動態が不安定なAKI患者のためのCRRT

RRTを受けるAKIの重症患者では血行動態不安定が一般的で(約36〜70%)41、これは死亡リスクおよびAKI進行リスクの上昇と関連する可能性があります。42,43

既存の医学的エビデンスは、間欠的血液透析(IHD)や持続低効率血液透析(SLED)と比較して、CRRTの方が体液を除去しながら血行動態の安定を維持できる可能性があることを示唆しています。38,44,45


CRRTの費用対効果: CRRTは、IRRTと比べ費用対効果の高い治療となる可能性があります。

 

AKI患者の初期治療としてCRRTを用いることは、RRT開始後5年時点で他のRRTモダリティと比べて、より費用対効果が高い可能性があります。5 

費用対効果分析は、透析依存率が主な要因であり、IRRTと比較して費用対効果が高い可能性を示唆している。46
